Hospice Support
Hospice care is sometimes referred to as end-of-life care and is a type of palliative (comfort) care for patients who are in their last stages of a serious illness and have opted not to continue with curative treatments. The purpose of hospice support is to focus on comfort, symptom/pain relief, and emotional-spiritual support.
Hospice care can be overwhelming to clients and their families. Hospice and Palliative Care are Medicare-covered services, but do not cover non-medical caregiving aspects. Connections non-medical services work in tandem with the medical hospice team. We can provide hospice trained care professionals to add additional support to the aspects of needed care that hospice or palliative care services do not cover; mainly aspects of the patient’s activities of daily life. These aspects include but are not limited to:
- Bedbound care
- Assistance with dressing
- Monitoring and recording intake and output
- Charting daily occurrences and care notes
- Personal hygiene: skin, hair, oral and nail care
- Incontinent care, brief changes
- Medication reminders
- Respite care for the family/primary caregiver(s)
- Light housekeeping such as laundry, bed making, and doing dishes
Care can be accommodated from four hours a day up to 24/7 care.
