Dementia Care
Dementia and Alzheimer’s Care
Dementia is a serious illness that can easily take its toll on family members. The most common and rapidly progressing form of dementia is Alzheimer’s disease. Some clients become confused, agitated, impulsive and experience social isolation to name a few.
Our trained care professionals not only go through an entire Dementia Program training circuit but have the patience and kind heartedness to handle these types of sometimes challenging situations, providing the right level of individual care and support while helping to form a regular routine and carefully monitoring aspects from medications being taken to daily activities, regardless of the level of cognitive and/or physical decline.
Connections owners will work with you to create a customized home care plan that encompasses the right mix of our service aspects that adapts to your loved one’s changing needs. Care plans are built after comprehensive analysis of your loved one’s abilities, interests, cognition and health status and are updated on an ongoing basis. Our goal is to focus on what your loved one is still able to do and bring as much meaning and purpose into their life as possible.
